
Understanding Chronic Post-Sternotomy Pain after Cardiac Surgery
A Prospective Study from Uppsala, Sweden
Introduction
Cardiac surgery has undoubtedly revolutionized the treatment of heart diseases, saving countless lives and improving patient outcomes. However, like any major surgical procedure, it can have its own set of complications and side effects. One such complication is chronic post-sternotomy pain, which can significantly impact a patient's quality of life. In this blog, we will delve into a recent prospective study that sheds light on the incidence and characteristics of chronic post-sternotomy pain after cardiac surgery.
The Study
The study, conducted by a group of researchers, aimed to determine the incidence of chronic post-sternotomy pain in patients who underwent sternotomy for cardiac surgery. A total of 349 consecutive patients were evaluated one year after surgery using a postal questionnaire that asked them to describe and score any persistent pain experienced following the surgical procedure. The patients were classified into three sub-groups based on the specific surgical procedure they underwent.
Findings
Out of the 318 patients who responded to the questionnaire (91% response rate), 90 patients (28%) reported experiencing chest discomfort that differed from what they had experienced before surgery. When assessing the pain intensity using the visual analogue scale (VAS), 41 patients (13%) reported a maximum pain intensity of 30 mm or higher, indicating moderate pain. Among these patients, 14 (4%) scored 54 mm or higher, indicating severe pain. Interestingly, there was no statistically significant difference in pain incidence and intensity when comparing patients who underwent different surgical procedures.
Implications
This study highlights the prevalence of chronic post-sternotomy pain after cardiac surgery, with 28% of patients experiencing some form of discomfort. While the majority of patients reported modest pain intensity, a small percentage (1%) experienced severe pain that did not fall below 54 mm on the VAS. These findings emphasize the need for healthcare professionals to be vigilant in assessing and managing post-sternotomy pain to enhance patients' post-operative experience and overall well-being.
Conclusion
Chronic post-sternotomy pain is a significant concern following cardiac surgery, affecting a considerable number of patients. While the majority of individuals experience moderate pain, a small proportion endures severe pain, impacting their daily lives. This prospective study highlights the need for further research and improved management strategies to minimize the occurrence and intensity of chronic post-sternotomy pain. By prioritizing patient comfort and well-being, healthcare providers can enhance the overall experience and outcomes of cardiac surgery patients.
